Impacts of OBBBA Implementation and Reduced Federal Health Care Spending

As implementation of the One Big Beautiful Bill Act (OBBBA) begins, the scope of its fiscal effects on stakeholders continues to emerge. Among its most controversial provisions is an estimated $1 trillion in Medicaid reductions—one of the most significant contractions in federal health care spending in recent decades.

While the law ultimately avoided some of the more far-reaching structural changes initially under consideration, the OBBBA ushered in several major changes that will reshape how states finance and administer their health care programs, including new restrictions on state-directed payments (SDPs), revisions to provider tax structures, the introduction of Medicaid work requirements, and a “Rural Health Transformation Program.”
